COLOR GUARD: Perry High School will be holding clinics on Tuesday, May 19 and Wednesday, May 20 after school in the high school front lobby for current 8th grade and high school students wishing to try out for our Color Guard next fall. The clinics will begin at 4pm in the front lobby and end at 6pm in the PAC gymnasium. Auditions will be held on Friday, May 22 in the HS Band Room. Please see Mr. Hruska if you have any questions.
SIDEWALK CHALK NEEDED: If you have any sidewalk chalk lying around at home that you do not need, the middle school art department would love to take it off your hands. Feel free to send it in with a student or drop it off at our office. Thank you and have a great day.
GRADE 8 CLAP-OUT: Our annual grade 8 clap-out will take place on the last day of school, June 1 at approximately 10:15 AM. Parents may enter through the high school and line up by the high school cafeteria to watch as students cross the bridge.
CLINIC REMINDERS:
- Medications stored in the clinic must be picked up by an adult by the last day of school, Monday, June 1st by NOON. All expired medication will be discarded.
- A new medication form must be completed for any medication for the upcoming school year. Prescriber Medication Forms are available on the Perry Local Schools website under> Department > Clinic > Form tabs are located on the right side of page or forms are available in the clinic.
- If your child has had any further immunizations since enrolling in kindergarten, please send a copy of your child’s immunization record to the clinic.
- The Ohio Department of Health requires Tdap and Meningococcal vaccines for all 6 grade students prior to entry to 7 grade, please forward proof of administration of these vaccines to the clinic.
- The Ohio Department of Health requires a Meningococcal vaccine for all current 11 grade students prior to entry to 12 grade, please forward proof of administration of these vaccines to the clinic.
KIDS EAT FREE: There is no registration and it is for age 18 or younger.
